We understand that the world outside of special education can be complex and difficult to navigate. We are here to help.
Along with the input from people with developmental disabilities and their families, we offer this compilation of resources to identify needs and set goals for students, families and educators towards the transition to adulthood and the future.
During transition, it is important to find ways to be valued and a contributing member of society. You need opportunities for making choices, learning new skills, building community connections, increased responsibility, and communicating your wants and needs as you transition to adult life. This tool will help identify a plan and provide additional resources for students with and without disabilities as they transition from youth to adulthood.

In May of 2009, Warren County was awarded a two-year, U.S. Department of Education's Safe and Drug Free Schools Grant to Integrate School and Community Mental Health Systems. Warren County was the only Ohio recipient of this Grant cycle and one of only 17 Grants awarded in the United States. The acquisition of "The Grant" was the result of the collaborative efforts of the ESC, all eight Warren County School districts, the Mental Health and Recovery Services and Centers of Warren/Clinton Counties, the Juvenile Court systems, and the generous work of many other community agencies and members.
The DSAGC is a 501C-3 non-profit organization established in 1981 with a history of responding to the changing needs of individuals with Down syndrome.
Our Cincinnati Autism Connection Information & Referral call and email response staff guide family members through the often confusing questions families face. You can connect to an actual person familiar with autism and available local tristate resources. Both the trained specialist and certified Information and Referral specialist are ready to assist you with autism information and local resources.
Special Needs Resource Directory: When you have a child with special health needs, you face the constant challenge of finding information and resources. The Special Needs Resource Directory, supported by Family Relations at Cincinnati Children's, can assist parents, caregivers and healthcare providers identify, evaluate and access information, services and supports. Focused on children with special health needs, the directory connects to local, state, national and international websites to help you.

The Cincinnati Recreation Commission’s Division of Therapeutic Recreation (TR) offers a continuum of leisure services for individuals with disabilities that promote a healthy lifestyle, as well as the opportunity to participate in high-quality recreational and cultural experiences in the community. As advocates for individuals with disabilities, we provide inclusive and therapeutic recreation programming choices and promote accessibility at all CRC facilities. Many of our full-time CRC Therapeutic Recreation Program Staff are Certified Therapeutic Recreation Specialists (CTRS).

Butler Behavioral Health was founded in 1948 to provide high quality behavioral health services in your community. Today, our founding values continue to assist individuals and families with a wide-range of needs with innovative and compassionate care.
The Transition Medicine team at Cincinnati Children’s wants to partner with teenagers and young adults with chronic health conditions or developmental disabilities to create a plan for healthcare transition as they get older. Patients as young as 16 years old can be seen by members of the Transition Medicine team.

Transition of Youth and Young Adults with Intellectual Disabilities (TrYAD) is a multidisciplinary clinic dedicated to helping teens and young adults with mild to severe intellectual disabilities transition to adulthood. TrYAD promotes self-management, advocacy, and continuity of care through a graduated transition process which involves:
• Education of patients, families, and their health care providers
• Coordination of ancillary services
• Liaison with community resources

Cerebral Palsy Guide is a national support organization dedicated to supporting anyone who has been affected by birth injuries, brain injuries, cerebral palsy, Erb’s palsy, and other developmental disabilities. We strive to provide answers and guidance to ensure that families receive the assistance that they need to help improve their overall quality of life.
