We understand that the world outside of special education can be complex and difficult to navigate. We are here to help.
Along with the input from people with developmental disabilities and their families, we offer this compilation of resources to identify needs and set goals for students, families and educators towards the transition to adulthood and the future.

 Steps to take by age level

During transition, it is important to find ways to be valued and a contributing member of society. You need opportunities for making choices, learning new skills, building community connections, increased responsibility, and communicating your wants and needs as you transition to adult life. This tool will help identify a plan and provide additional resources for students with and without disabilities as they transition from youth to adulthood.
